Complaints

The Practice will take seriously any complaint recieved from members of the public and will acknowledge all complaints within 3 working days of receipt.

If you would like to request a copy of the Practice's Complaints procedure, please enquire at reception, or alternativley email: info.collierswood@nhs.net. Please note the Practice has the right to charge for such requests.

If you would like to make a complaint you can use the form below to do so.

If you wish to make a complaint on behalf of someone else you will need to download the Third Party Consent Form after completing the online form.
